trans-scaphoid and trans-capitate perilunate fracture-dislocation of the wrist with concomitant ipsilateral fractures: a case report

چکیده
absractall perilunate fracture-dislocations combine bone avulsions, ligament disruptionand fractures in different forms. the most frequent pattern is the dorsal transscaphoidperilunate fracture-dislocation. the reported risk of missed initial diagnosisin perilunate dislocations is as high as 20%. concerning pathomechanics of theperilunate fracture-dislocation, it should be noted that most dorsal injuries usually resultfrom a fall on the outstretched hand or motor vehicle accidents although othermechanisms are also mentioned. we describe here a case of dorsal trans-scaphoidtrans-capitate perilunate fracture-dislocation together with posterior olecranon fracture-dislocation of the left upper extremity. to the best of the authors' knowledge, such a case with this concomitant elbow fracture-dislocation has not been previouslyreported in the literature.

Of all wrist dislocation, the perilunate is the most common type of dislocation. There is ligament disruption typically begins radially and propagates around the lunate. It may be associated with different carpal fractures which the most common pattern is the trans-scaphoid perilunate fracture-dislocation [1,2] which constitutes approximately 50% of these injuries [1-5]. Injury to the distal ra...

Rowan CH, Downing ND. BMJ Case Rep 2017. doi:10.1136/bcr-2017-221041 Description Perilunate dislocations are uncommon high-energy injuries which are missed in approximately 25% of cases on initial presentation. We present the X-rays of a 25-year-old man who fell from a height of approximately 2 metres onto an extended wrist. Plain radiographs (figure 1) show classical signs of a trans-scaphoid ...
متن کاملBilateral Trans-Scaphoid Perilunate Fracture Dislocation
The wrist represents a complex anatomic region in the human extremity and a highly functional and intricate structural joint. Most perilunate injuries result from a staged disruption of the ligamentous structures. Forces may be directed through the carpal bones themselves resulting in a fracture. Early anatomic reduction of any carpal malalignment is of the utmost importance. Unfortunately, eve...
متن کاملTrans-Scaphoid Perilunate Fracture-Dislocation and Isolated Perilunate Dislocations; Surgical Versus Non Surgical Treatment
Background: Trans-scaphoid perilunate fracture-dislocation and perilunate dislocations are among uncommon injuries, most commonly seen in young patients due to high energy trauma. The treatment can be achieved either surgically by open reduc tion and internal fixation or closed reduction and casting. Methods: To compare surgical versus non-operative results of treatment of trans-scaphoid pe...
متن کاملTrans-radial styloid, trans-scaphoid, trans-triquetral perilunate dislocation.
A rare case of trans-radial styloid, trans-scaphoid, trans-triquetral perilunate dislocation is described. The injury was treated by reduction and internal fixation using a Herbert screw and Kirschner wire. A good functional result was achieved. A review of the literature illustrates that early surgery is recommended. Alternative surgical approaches are discussed.
